Connexu Team now does Path Plans

Exciting news! Our ConneXu Outcomes Team have been trained to complete Visual Plans for ConneXu partners. In the past, external provider Interactionz exclusively did these, but after receiving training and the go-ahead from them, we are now able to complete them in-house as well, when capacity / turnaround times are an issue at Interactionz and they need our back-up support.

A Visual plan such as a PATH or MAP identifies your interests and what you want to do in life and then sets steps and timeframes to be able to move through that pathway.

The Outcomes Leads along with the Outcomes Team lead have been trained to facilitate the plans. Wesley says they follow a very thorough process to do a plan.

“We spend a whole day doing a PATH Plan and involve the people that the disabled partner wants to involve, including family / whanau, Community Facilitators and Leaders,” says Wesley.

“It’s about getting a picture of who the person is and who can be involved in supporting them to achieve their goals. We breakdown the goals in various timeframes, for example the next few days, six months, two years and the bigger picture.”

“One PATH Plan will include various goals, with specific timeframes and who is responsible for supporting. It is intended to give you a guide and pathway to move forward.”

Robert Peachman is the first Disabled partner to have their Path Plan completed by the Outcomes Team in Tauranga.

“Robert’s visual plan is now on his wall and he can refer to it and see it as he moves forward.”

“He is very interested and involved in bird watching and wetland conservation. One of his goals is to visit the Miranda Shorebird Centre to see the birds migrating in that fly all the way from Alaska.”

“We’re working towards a trip there in his plan, where ConneXu can support with transport and staffing to make that happen. That’s what it’s all about.”
