The Role

We have a great opportunity to join our Service Delivery team as a Community Mentor. This is a community-based role working within ConneXu residential support services. The Community Mentor is responsible for assisting people we support and their networks (paid, volunteer and natural) to identify, plan for, and achieve goals.

Working alongside community facilitators, volunteers, and other natural supports to work toward individual progression to live and breathe the eight principles of enabling good lives and align all practice to these under the direction of the Residential Practice Manager.

Your key responsibilities will include:

  • Supporting people with different abilities to become active and valued members of their community

  • Facilitation of goal planning, reporting on goals and Whole of Life Plans

  • Collaboration with teams and motivating relevant parties to understand quality of life and EGL

  • Providing additional support through transitional pathways

  • Assisting with volunteer referral documents

The Person

We are looking for a person who has a passion for working with others and a drive to Enable Good Lives. You must have previous experience working with vulnerable people and a proven ability to partner with others. You will also be a critical thinker who is able to communicate well.
